The Witness Sample Declaration documents that the DNA sample was collected from the correct animal and that the animal’s identity was verified by an independent witness at the time of sample collection.
The witness confirms that the submitted DNA sample was collected from the animal specified on the submission form. This helps ensure the reliability of sample identification.
Please note: The witness must be present during sample collection and must complete and sign the declaration in full for us to be able to process it. The completed and signed form must be physically sent to the laboratory together with the DNA sample. A digital copy is not sufficient.
